Key Reports and Assessments to Prepare


Here are the most commonly recommended reports we provide for NDIS plan reviews:



  1. Functional Capacity Assessments (FCA)


Written by occupational therapists, these assessments give the NDIS a clear picture of how your disability impacts daily life. They cover mobility, personal care, cognition, community access, home tasks, and equipment needs.


Recommended for:


  • Plan reviews

  • Change of circumstances

  • New or increased support needs

  • Transition to Supported Independent Living (SIL), ILO, or SDA




  1. Progress Summary Reports


These mid-plan or end-of-plan updates outline the therapy you’ve received, progress made, and ongoing support recommendations.


Provided by:


  • Occupational therapists

  • Speech pathologists

  • Allied health assistants (in collaboration with supervising therapists)


Recommended for:


  • Demonstrating therapy outcomes

  • Justifying continuation or increase of services




  1. Assistive Technology (AT) Reports


These include trials and justification for items such as shower chairs, wheelchairs, beds, hoists, or communication devices. Our OTs help submit quotes and risk assessments.


Recommended for:


  • Requesting new or replacement equipment

  • Low-cost, mid-cost, or high-cost AT applications



  1. Sensory Profiles


For participants with autism, trauma backgrounds, or regulation challenges, a Sensory Profile helps identify needs around emotional regulation, environmental adjustments, and support strategies.


Recommended for:


  • Therapy justification

  • Positive Behaviour Support plans

  • Classroom or home strategies




  1. CELF-5 and Communication Assessments


Our speech pathologists assess language development, comprehension, expression, and social communication—often essential for school-aged clients.


Recommended for:


  • Children with learning difficulties, autism, ADHD

  • Literacy or social skill challenges

  • Supporting requests for ongoing speech therapy




  1. Housing Reports for SIL, ILO or SDA


If you’re exploring housing support options, our OTs provide in-depth reports to support eligibility and identify environmental or supervision needs.



When Should You Start Preparing?


We recommend starting 6–8 weeks before your plan end date to allow time for:


  • Therapy sessions and assessments

  • Collaboration with your support team

  • Report writing, reviews, and finalisation

  • Submission to the NDIA before your review meeting
